This opportunity is for a Transition Coordinator (LPN) for our Hampton Rehab team. This Department of Riverside Convalescent Center- Hampton provides outstanding care to patients with excellent patient quality of care outcomes. The selected Licensed Practical Nurse will be part of a team that works in a supportive and collaborative culture that with others, provides competent and compassionate care to all patients and their families.
Works under the supervision of the Central Transition Director. Performs coordination of patient services to provide a seamless transition from acute settings to Lifelong Health. Elements required for above include data collection and clinical review of all referrals; determination of referral acceptance based on established criteria and collaboration with various team members; attaining and remaining knowledgeable and current related to Medicare/Insurance reimbursement and basic coverage criteria and of service lines and products; and conferencing with patients families discharge planners physicians medical staff and others to coordinate and facilitate transition and services. Works closely with facility care management/discharge planners to identify potential referrals for Lifelong Health Services. Provides exceptional customer service and serves as the face of Riverside Post Acute Care Services to all contacts by telephone in person and via electronic media.
Riverside began serving the Virginia Peninsula community in 1916. In 1963, Riverside moved to the present 72-acre location in Newport News on J. Clyde Morris Boulevard. A 450-bed facility, Riverside Regional Medical Center is the Virginia Peninsulas most comprehensive medical facility combining the most recent computerized medical technology with a healing environment. The Pavilion is a 250,000 sq. ft., $107 million, 3-story addition to Riverside Regional Medical Center, which opened in 2013. This acute care facility provides a regional cancer care center, the Peninsula regions only comprehensive stroke center, the areas only open-heart surgery program, the regions only Level II Trauma Center, and a cornerstone neurosciences program that includes the Chesapeake, Riverside and University of Virginia Radiosurgery Center.
EDUCATION: Minimum requirement of LPN Licensure requirement for the State of Virginia or a LPN from an accredited school of nursing and licensed in the State of Virginia.
EXPERIENCE:Minimum of 5 (five) years in nursing experience and 2 (two) years in Home Health, Home Infusion, Hospice or Discharge Planning preferred.
LICENSURE/CERTIFICATION: Licensed as a LPN in the State of Virginia without restriction required. Must also have reliable transportation to make visits to assigned locations each day.
