The office assistant works under the direct supervision of the office manager. Primary responsibilities include insurance eligibility patient registration and reconciliation within the patient registration software properly entering patient demographics as well as greeting patients while answering the phone lines. Also responsible for taking appropriate phone messages scheduling appointments creating referrals completing chart prep providing information to and for the patients maintenance and upkeep of the reception area distributing mail and assisting other employees as needed. The office assistant also identifies corrects and reports compliance insurance and billing anomalies as they are recognized. Responsible for all benefit collections; charge reconciliation payment posting insurance verification and authorization. Accountable for customer registration in iCare. Responsible for contacting patients on the Referral Workqueues and place notations as needed. Functioning as a team member the staff member is required to be flexible in their work assignments and at times may be asked to perform other duties as assigned. Must be available for rotating schedules as well as some light traveling to support all aligned rehab service facilities when needed. The staff member will also assist with new employee on-boarding training. Responsible for the maintenance of office equipment and supplies required to support the facility operations. In addition the staff member will comply with National Patient Safety Goals HIPAA guidelines and Corporate Compliance polices. Participates in quality and customer service programs and meets all CBL and renews their CHAA Certification timely. The Clinical Pharmacist-Ambulatory Care is responsible for medication management of patients in the ambulatory setting through a collaborative model that includes clinic providers, clinic staff and care management. This includes disease state management, patient education, optimization of drug therapy regiments, medication reconciliation, high-cost drug therapy management and other initiatives designed in collaboration with health system pharmacy leadership, clinic providers and staff. The Clinical Pharmacist-Ambulatory Care will serve one or more medical group practices. Essential Functions and Duties include: Manage patients’ chronic disease states via collaborative drug therapy management (CDTM) in conjunction with providers and extended care team. CDTMs include, but are not limited, to the following conditions: Diabetes, asthma/COPD, depression, hypertension, anticoagulation, congestive heart failure, medication adherence and other chronic diseases. Actively participates in drug reconciliation and transitions of care, including transfers between home/long term care, acute inpatient care and other sites as requested. Provides telephonic and in-person disease state management services to include complex/polypharmacy medication review, drug-drug interaction checking, patient education and interventions to improve medication therapy adherence. Conducts comprehensive and targeted medication review for pre-defined populations and in accordance with contractual requirements. Participates in the development, implementation and measurement of outcomes of disease state management initiatives in the ambulatory care setting. Works as part of a multidisciplinary team to provide patient-centered care as part of the Primary Care Medical Home in conjunction with Care Coordinators, Providers, Health Coaches and other clinic staff. Assures appropriate documentation of drug therapy interventions in patient’s medical record. Documents all patient care with proficiency and in compliance with health system policies and regulatory agencies. Provides ongoing education on medications to patients, providers and clinic staff Provides timely interventions to optimize flow within the clinic setting. Maintains strong rapport with health care practitioners. Develops and maintains knowledge of federal and state regulations, basic insurance and benefits knowledge. Provides support to meet reporting requirements of organization’s contractual obligations in terms of pharmacy compliance and monitoring. Conducts all activities with the highest standards of professionalism and confidentiality. Complies with all applicable laws, regulations, policies and procedures, supporting the organization’s corporate integrity efforts by acting in an ethical and appropriate manner.
